FCRA Disclosure & Authorization – Important – Please Read

In connection with your application for employment (including contract for services), Hylas Companion and its agents may obtain consumer reports including MVR, PSP, DAC, Clearinghouse, and criminal background. You authorize all previous employers, schools, and agencies to release information. This is per FMCSA regulations 49 CFR 391.23 and 49 CFR 382.

I certify that this application was completed by me and all entries are true and complete to the best of my knowledge. I understand false statements may lead to disqualification or termination.

By signing below, you authorize procurement of consumer reports and agree to FMCSA Clearinghouse query.

I authorize FMCSA Drug & Alcohol Clearinghouse full query and understand results will be reviewed.

I authorize Hylas Companion to obtain MVR, PSP, DAC reports and contact previous employers per §391.23.

I authorize previous employers to release all DOT safety performance history including drug/alcohol information.

Authorization for Third-Party Submission – Limited Power of Attorney

I authorize Hylas Companion, its agents and assigns, to act as my authorized representative and preparer for the purpose of submitting employment applications for truck driving positions on my behalf to motor carriers and their service providers (including Tenstreet, IntelliApp, Driver Pulse).

I understand Hylas Companion may transfer information provided in this application, including my personal data, employment history, and safety history, to such carriers to facilitate my employment.

I authorize Hylas Companion to affix or apply my electronic signature, as drawn/typed below, to such applications and related FMCSA-required authorizations, and I agree that such signature shall have the same force and effect as if I had personally signed the carrier's application. I retain full responsibility for the truthfulness of the information.

This authorization is valid for 1 year from the date below and may be revoked in writing at any time. I have read and agree to the E-SIGN disclosure that electronic signatures are valid. IP and timestamp will be recorded.

Exclusivity Agreement

Do I pay anything to get started?

Never. Drivers pay zero — no application fees, no placement fees, no startup costs. Hylas Companion is paid by the carriers we place you with.

Do you hold my first paycheck?

No. Settlements are processed weekly with no held pay. You'll know your exact pay schedule from day one.

When will a recruiter call me?

Our goal is to call within 1 business day of your application — usually the same day during business hours. We call. We don't ghost.

Do I need prior OTR experience?

Not for our Trainee Program. You'll ride with a seasoned trainer to build your road hours, then move to a solo seat. CDL-A required.

Do you welcome female drivers?

Absolutely. We recruit based on qualifications only. Every position we offer is open to all CDL-A licensed drivers equally.

What if I have violations or accidents?

Apply anyway and be upfront. Our recruiters match you with carriers whose safety standards fit your record. We won't waste your time or theirs.
